For liability reasons, the University cannot allow any child to participate in any Children's Day activities without signing the appropriate consent forms.

Hard copies of all consent forms (and copy of medical insurance card) MUST be sent to Wendy Nishikawa, Co-Chair of Children’s Day, at Human Resources (mail code 3160) by 5:00 p.m. on Friday, March 23, 2007.
